Insight Global is seeking a Clinical Nurse Manager to support one of our top home health clients in their Elmsford, NY office! This full-time role offers an exciting blend of fieldwork and office-based responsibilities, empowering you to lead with compassion, expertise, and innovation while supporting the care plans and health of your clients.

Responsibilities

• Lead and manage nursing staff to ensure high-quality patient care.
• Coordinate patient care plans and collaborate with interdisciplinary teams.
• Conduct regular assessments and evaluations of patient needs.
• Ensure compliance with healthcare regulations and standards.
• Provide training and support to nursing staff.

Fieldwork (30-40%)

• Conduct monthly client visits, managing a caseload of 15–30 clients with personalized care.
• Support new client admissions and perform thorough assessments to ensure tailored care plans.
• Develop and update individualized care plans to meet evolving client needs.
• Supervise and mentor field staff, fostering their professional growth and ensuring exceptional care delivery.
• Serve clients across northern Bronx and Westchester County, with occasional travel to Putnam, Rockland, and Orange Counties.

Office-Based Leadership (60-70%)

• Oversee and validate field staff competencies for new hires and annual reviews, ensuring top-tier performance.
• Review and maintain accurate, high-quality documentation for all cases.
• Lead engaging staff training sessions and in-service education to promote continuous learning and excellence.

Requirements:

• Registered Nurse (RN) license in the state of New York.
• Bachelor's degree in Nursing (BSN) preferred.
• Minimum of 2 years of nursing experience.

Preferred Qualifications:

• Experience in home healthcare or community health settings.
• Previous management or leadership experience.
