Full-Time RN Case Manager – Hospice Nurse (Spring, Tomball, North Houston)

What You’ll Do:

• Manage a caseload of hospice patients in Spring, Tomball, and North Houston.
• Provide skilled, compassionate nursing care and coordinate individualized care plans.
• Complete routine visits, admissions, recerts, and IDT documentation.
• Collaborate with the interdisciplinary team and participate in biweekly IDT meetings.
• Communicate effectively with patients, families, and providers.
• Maintain accurate, same-day charting with a provided tablet.
• Deliver quality end-of-life care that reflects our values of compassion and respect.

What We’re Looking For:

• Current RN license in Texas.
• Hospice or home health experience preferred (training available for the right candidate).
• Excellent communication, organization, and teamwork skills.
• Reliable transportation and willingness to travel locally within the service area.
• A passion for caring for people and making a meaningful impact every day.
